We gladly see patients who do not have one of our contracted insurance plans and we require payment at the time of service. We will submit one claim per visit for patients with plans that we do not contract with and the patient is responsibility for following up with their insurance company regarding reimbursement.
Fees for all cosmetic services, product sales and insurance co-pays are due at the time of service. We accept cash, check, Visa and MasterCard.
Patients are asked to provide at least a full business day's notice (24 hours) when cancelling or re-scheduling an appointment. Failure to provide 24 hours notice will result in a charge of $25 for an office visit appointment and $50 for a surgical procedure appointment. The practice reserves the right to discharge patients who have canceled an appointment with less than 24 hours notice in excess of 3 times.
New patients are asked to complete a patient registration form and medical history form at the time of their initial appointment. At every subsequent office visit and surgical appointment, patients are asked about changes in their medical history and medication list that may have occurred since their last visit. In addition, we ask existing patients to update their registration form every 2 years and their history form every 5 years. The registration and history forms can be downloaded and completed prior to your visit or you may arrive 10 minutes early for your appointment to allow time to complete them in the office.
