Directions:

From Santa Rosa take Hwy 12 into Sebastopol. Turn Left on to S. Main Street, through downtown Sebastopol. S. Main Street turns into Hwy 116 - (stay in the Right lane). Turn Left onto Corline Court (Apple Valley Rehabilitation is on the corner). Our parking lot is the 4th driveway on the left and our building is at the very back of the parking lot, #C.

From Petaluma/Cotati take Hwy 116 into Sebastopol. Turn Right onto Corline Court (this is 2 blocks past the Holiday Inn.) Our parking lot is the 4th driveway on the left and our building is at the very back of the parking lot, #C.

Insurance Information

We have contracts with select PPO insurance plans and Medicare. If you have one of these insurance plans then our office will submit a bill to these companies for the medically necessary services that you receive in our office. You will receive a bill if there is a balance after you insurance company has paid their portion. Any services falling under your annual deductible, however are payable at the time of your appointment.

Our current contracts are with the following plans:

  • Medicare
  • Blue Cross PPO
  • Blue Shield PPO
  • Cigna PPO
  • First Heath
  • Interplan

We gladly see patients who do not have one of our contracted insurance plans. We require payment at the time of your appointment and will give you a receipt itemizing the visit and services you receive (a medical 'superbill') and you can submit it to your insurance company. We unfortunately cannot complete your insurance paperwork for you and it is your responsibility to do this if you would like reimbursement from insurance companies that we do not contract with.

Fees for all cosmetic services, product sales and insurance co-pays are due at the time of service or purchase. For your convenience we accept cash, check, Visa, MasterCard, Discover and American Express.

We contract with BillRite Medical Billing Services for our insurance billing and claims submission. Questions regarding insurance billing and patient account statements can be addressed to Brent or Terry Anderson at 707-838-2711.

Cancelation and 'No-Show' Policy

Our cancelation and 'no-show' policies and fees vary by appointment type which reflects the amount of time reserved for you and the administrative time needed to book another patient into your appointment time.

General Appointment: We require 24 hours cancelation notice for general/medical appointments. If your appointment is cancelled less than 24 hours prior to your appointment time or if you fail to show up to your appointment all together, you will be charged a $75.00 fee.

Surgery Appointment: If your appointment is cancelled less than 48 hours prior to your appointment time or if you fail to show up to your appointment altogether, you will be charged a $150.00 fee.

Cosmetic Treatments: Our office holds all cosmetic appointments with the patient's credit card. If your appointment is cancelled less than 48 hours prior to your appointment time or if you fail to show up to your appointment all together, you credit card will be charged the amount of the service in full.

REMEMBERING YOUR APPOINTMENT IS ULTIMATELY YOUR RESPONSIBILITY. AS A COURTESY, WE USE AN AUTOMATED APPOINTMENT REMINDER SYSTEM THAT MAY CALL YOU 48 HOURS PRIOR TO YOUR SCHEDULED APPOINTMENT. PLEASE KEEP US INFORMED OF YOUR CURRENT TELEPHONE NUMBER.

The practice reserves the right to discharge patients who have canceled with less than 24 hours notice or 'no-showed' for their appointments in excess of 3 times.

Patient Registration Process

New patients are asked to complete a patient registration form and medical history form at the time of their initial appointment. At every subsequent office visit and surgical appointment, patients are asked about changes in their medical history and medication list that may have occurred since their last visit. The registration and history forms can be downloaded and completed prior to your visit or you may arrive 15 minutes early for your appointment to allow time to complete them in the office.
